A recent FENIX Group analysis published by Fierce Pharma evaluated more than 20 companies across factors including pipeline depth, clinical differentiation, and commercial infrastructure to identify the pharmaceutical companies best positioned to compete.

Eli Lilly and Novo Nordisk remain the clear leaders, ranking first and second, respectively. Lilly’s tirzepatide franchise generated more than $36 billion in revenue in 2025, while the company prepares for the launch of its oral obesity treatment, orforglipron. Novo Nordisk, meanwhile, is relying on new formulations of Wegovy and CagriSema as it works to maintain its position in an increasingly competitive market.

The rankings also highlight several companies emerging as serious challengers:

  • Pfizer ranks third following its acquisition of Metsera and licensing agreement with YaoPharma, giving the company a more differentiated obesity pipeline and the commercial infrastructure to support it.
  • Roche ranks fourth and has set its sights on becoming a top-three player, with promising clinical programs including CT-388 and petrelintide.
  • AstraZeneca is viewed as a potential “dark horse,” supported by a broad mid-stage pipeline and multiple anticipated clinical readouts.
  • Amgen is betting on MariTide and its potential advantage of extended dosing, although tolerability remains an important consideration.
  • Boehringer Ingelheim, Regeneron, Zealand Pharma, and Kailera Therapeutics round out the top 10, each pursuing differentiated approaches to obesity treatment.

What This Means for the Life Sciences Industry

The next phase of the obesity market will likely be about more than developing an effective weight-loss drug. Manufacturing capacity, commercial infrastructure, strategic partnerships, pipeline breadth, and the ability to differentiate therapies will all play critical roles in determining which companies can compete at scale.
